NPS Survey
Add a 0–10 Net Promoter Score field to any Gravity Form and view aggregated NPS reports with shareable links.
What it does
Collecting customer satisfaction feedback usually means sending people to a separate survey tool. This add-on lets you add a dedicated NPS field directly to any Gravity Form, so respondents stay on your site and their scores flow straight into your WordPress entries.
When someone submits the form, the add-on saves their score, automatically labels them as a Promoter (9–10), Passive (7–8), or Detractor (0–6), and makes both the score and category available in conditional logic, merge tags, notifications, and confirmations. You can show follow-up questions based on the score—for example, asking Detractors what could be improved or inviting Promoters to leave a review.
The results page shows your calculated NPS, response mix, category totals, score distribution, and performance over time. When you need to share results, you can generate a secret link that lets anyone view the live aggregated report without logging in to WordPress.

Good for
Written from Gravity Forms’s own listing. Version and dates come straight from the source and are checked weekly.
What scale styles are available for the NPS field?
You can choose from four styles: Buttons, Circles, Radio Buttons, and Spectrum.
Can I use the NPS score to trigger different form fields or notifications?
Yes. The score works with Gravity Forms conditional logic, so you can show or hide fields based on the submitted score, and you can include the score or category label in notifications using merge tags.
What does the shared report show, and does it expose individual entries?
The shared report shows the overall NPS, response mix, category breakdown, trend over time, and score distribution. It does not display individual entries or contact details.
What are the minimum requirements to run this add-on?
You need Gravity Forms 2.9.24 or newer, WordPress 6.5 or newer, and PHP 7.4 or newer.
